EFT-Term 6 months – Home Care Coordinator/Nurse

JOB SUMMARY: The Coordinator will be required to coordinate and manage the established Home Care and Community Care Program and the Homemaking Program for the community of Fisher River Cree Nation. The Home Care Coordinator/Nurse will work within a multi-disciplinary team to receive referrals, assess care needs, determine eligibility for the HCCP/HM Programs, develop care plans and coordinate the implementation and evaluation of a broad range of services based on the HCCP guidelines.
QUALIFICATIONS:
Current C.R.N.M. (Canadian Registered Nurses/Manitoba), Bachelor of Nursing Degree preferred with good standing of registration with advanced training and/or experience in nursing and management;
Also accepting Nursing Diploma in Licensed Practical Nursing, actively practicing membership with the College of Licensed Practical Nurses of Manitoba (CLPNM)
Current CPR/FA/AED and ITLS;
Maintain a professional code of ethics when applying direct services to members and their families;
Knowledge and experience in developing and implementing care plans and reports and managing budgets;
Understanding of the First Nations Health Care Delivery and Services of the HCCP;
Ability to provide direction and support to direct service providers;
Provide direct nursing care to clients as required;
Knowledge of FRCN traditions, lifestyle and language would be an asset;
Ability to work as a team leader and work independently;
Excellent organizational and motivational skills;
Able to take direction and follows through to completion of tasks;
Excellent communication and writing skills;
Excellent physical, mental and emotional health;
Proficient in office skills and computerized systems (Windows-Word, Excel, PP);
Must abide by Fisher River Cree Nation’s Policies and By-laws;
Adhere to Criminal Record Check, Adult Abuse and Child Abuse Registry and testing for Illegal Drug;
Must have own vehicle and a valid Manitoba driver’s license;
Must be available to work flexible hours and to travel when required.
