Under the general direction of Nurse In Charge, the Diabetes Support Worker will work in a multidisciplinary environment utilizing internal and external services to meet the needs of members who are seeking support for awareness, education and diabetes care.


 Certificate in Diabetes Education or be willing to undertake training in diabetes education and/or Certified Health Care Aide;
 Experience facilitating workshops/group sessions and one-on-one client health education;
 Facilitate and maintain strong prevention and awareness
sessions/workshops/groups and/or individual teachings;
 Must possess a professional code of ethics when providing direct services to members and their families who are seeking local and external services;
 Food Handlers Certificate or willing to train or update;
 Excellent physical, mental and emotional health;
 Ability to work as a team member and also work independently;
 Excellent communication and written skills;
 Proficient in office skills and computerized systems (Windows 7, Excel, PDF, Power Point);
 Basic budgeting and work plans knowledge;
 Some knowledge of eChart and Telehealth, or willing to learn;
 Own a vehicle and possess a valid Manitoba Driver’s License;
 Must be willing to travel when required;
 Must adhere to a Criminal Record Check and Child/Adult Abuse Registry;
 Must be willing to abide with the FRCN Illegal Drug By-law.


 Assisting & assessing referrals for the Diabetes Program through the direction of the NIC;
 Provide home visits to clientele as required for one-on-one education;
 Prepare education sessions/workshops/materials are per yearly work plan;
 Assist with school health education on topics such as nutrition, physical activity, and general diabetes topics;
 Set up weekly diabetic clinics with various topics/sessions;
 Complete and maintain accurate documentation;
 Arrange and coordinate dietician/nutritionist visits as per referral criteria;
