Manitoba is now offering an Immunization Card for people are fully immunized.

To request an Immunization Card, community members can call 1.844.MAN.VACC (1.844.626.8222) and follow the call prompts to request your card.

When you request your Immunization Card, a digital card that you can carry on your mobile phone will be created automatically.

If you request a physical card, it should arrive in the mail within two weeks.

These cards will include your first and last names and a QR code.

To be eligible to request your Immunization Card:

  • You must have a Manitoba Health Card
  • You must wait 14 days after getting you LAST dose of a COVID-19 Vaccine
  • You must have received all required doses of a vaccine, that means you require two doses.
