Immediate Supervisor:             Jordan’s Principle Director

Job Summary:   Under the direction of the Jordan’s Principle Director, the Medical Navigator provides comprehensive child services with the Jordan’s Principle team. As a specialized healthcare liaison, this role guides patients through complex medical care, coordinates treatment plans, and bridges gaps between general health and supportive services.

Key Responsibilities:

  • Schedule medical appointments, specialist referrals, and follow-up visits to ensure seamless care transitions.
  • Assist children with health needs and regional requests to ensure all supportive services are secured.
  • Serve as the primary point of contact to educate patients on medical treatment options, address anxieties regarding procedures, and advocate for children’s needs.
  • Collaborate with physicians, specialists, and other healthcare professionals to develop and implement individualized care plans.
  • Identify and connect patients with community resources.
  • Assess patients to identify appropriate nursing interventions and administer medications or treatments as prescribed.
  • Monitor, document, and report symptoms or changes in patient conditions.
  • Provide health-related counseling to patients and families.
  • Adhere to Fisher River Cree Nation Policies and Procedures, including the Illegal Drug By-law.

Qualifications:

  • Current BN/ RN/ LPN Manitoba License.
  • Experience and knowledge in the Paediatric field.
  • Familiarity with Provincial Standards or experience in a similar environment is an asset.
  • Strong management, communication, and human relations skills.
  • Professionalism, accountability for nursing practice, and strict adherence to confidentiality.
  • Strong knowledge of medical terminology, dental care procedures, and insurance structures.
  • Exceptional interpersonal and problem-solving skills, particularly with vulnerable clients.
  • Valid Manitoba Driver’s License and access to a personal vehicle.
  • Knowledge of Fisher River Cree Nation traditions and lifestyles.
  • Proficiency in Microsoft Office.
  • Proven ability to work effectively within a team and with external resources.
