Nursery to Grade 12 student form registration will begin August 7th to August 31st @ 8:30 a.m. to 3:00 p.m. daily at Charles Sinclair School.
Registration Form information required:
• Student’s Full Name
• Student’s DOB
• Names & Contact Information for Parents/Guardians
• Name & Contact Information for Emergency Contact
• Band & Treaty Number
• 6 & 9 Digit Medical Numbers
• Who is permitted to sign your child out of school
• Custody agreement/order in place
STUDENTS WILL NOT BE PERMITTED TO BEGIN CLASS UNTIL A NEW REGISTRATION FORM IS COMPLETED WITH ALL REQUIRED INFORMATION.

The Fisher River Health Centre has ongoing dental services with Dr. I. Bruce locally.
